Sports Physical Information:
**Forms will be available after May 1st when Final Form turns over for the new school year**
West Branch High School will be holding free sports physicals for the upcoming 2026-2027 school year.
Complete and sign all online forms thru Final Forms for the upcoming 2026-2027 school year. Be sure to
SELECT THE SPORT(S) YOUR STUDENT-ATHLETE PLANS TO PARTICIPATE IN. There is no obligation to join and their names can easily be removed if they choose not to participate. Choose all sports that may apply. All forms must be completed before you print the Physical form. Select
OHSAA PPE (Physical for Physicians) and print. Please
sign this paper form and send it with your student to school.
Current 6th and 7th graders:
- Tuesday, May 19th During lunch periods- Turn in the form and complete screening of height, weight and blood pressure.
- Thursday, May 21st - Physicals will be done at the Middle School. Students will be called down alphabetically to the big gym.
Current 8th-11th graders:
- Thursday, May 7th- During lunch periods- Turn in the form and complete screening of height, weight, and blood pressure.
- Friday, May 15th- Physicals will be done at the high school. Students will be called down alphabetically to the HS gym lobby.
